- Preventive services at little or no cost and low copayments for many dental services.
- Minimum 12-month participation required.
BlueDental Choice Copayment (PPO)
- Covers preventive services like check-ups, cleanings, and specialty services when visiting in-network dentists and specialists.
- No referrals or authorizations for care are ever needed for you to see a general dentist or specialists.
- No waiting period for preventive and basic services.12 month waiting period for major services.
BlueDental Choice Plus (PPO)
- Covers preventive services like check-ups, cleanings, and specialty services when visiting in-network dentists and specialists.
- No referrals or authorizations for care are ever needed for you to see a general dentist or specialists.
- No waiting period for preventive services, 6 month waiting period for basic services. 12 month waiting period for major services.
